WHAT HAPPENS DURING YOUR HEALTHY HOUSE CALLS VISIT?
- Visit with a Healthy House Calls care team member in your home who collaborates with your primary care provider in overseeing your health and wellbeing.
- The Healthy House Calls healthcare professional will review your prescriptions, medical history, review any health concerns, and ensure any screening and quality tests appropriate for you have been performed.

Your Healthy House Calls care team consists of a licensed advanced practice provider (nurse practitioner or physician assistant) and a community health worker who will provide you the support and guidance you deserve.
WHAT TO EXPECT?
After your in-home visit, the information gathered will be shared with your primary care provider who will work with you directly to manage and provide treatment for any health concerns found during your check-up. Your TriHealth primary care provider will stay in close contact with your Healthy House Calls care team as we collaborate to manage your healthcare needs.
